Medicare's skilled nursing benefit requires a qualifying inpatient hospital stay first -- time spent under observation status does not count, no matter how many nights were spent in a hospital bed. Long-term nursing home care is not a Medicare benefit at all. Nursing homes in NC are licensed separately from adult care homes and carry the federal CMS Five-Star rating.
Why the same building often houses both
Many NC nursing facilities operate rehab and long-term-care wings under one roof and one license, serving two very different populations: patients on a time-limited Medicare rehab stay, and residents in permanent, Medicaid- or private-pay-funded long-term care.
